[TNS-list] Emergency Contact Information Form and Family Survey

Dear TNS Community,

Every year, families are required to complete a DOE emergency, safety, and
health information form (aka the “blue card”). Because of remote learning,
it is more difficult to do this in person, so we have moved the form to an
online, secure link for you to fill out. This should take 5-10 minutes to
complete and hopefully is easier to do. You must fill out one form for each
child you have attending TNS.

Emergency, safety, and health information is confidential and will not be
shared outside of the DOE or TNS administration. Your contact information
(email, phone number, and address) will be added to a directory for
teachers, PTA, and other parents. The Parent Coordinator will use the
family survey to assist individual families and to recommend priorities and
goals for the PTA.



We also have some optional survey questions for you to fill out regarding
your TNS community needs and interests. This information will be critical
to us so that we can prioritize resources and energy. This is also an
opportunity to indicate to the PTA if your family is interested in online
or in-person activities outside of school, forming a learning pod with
other TNS families or other ideas as well as a way to get involved.
